Physicians join hands to provide care for the poor

For six years, impoverished patients in places as diverse as Nicaragua and Oklahoma City have turned to M. Boyd Shook, FACP, for medical care.

Dr. Shook is founder of the nonprofit organization Manos Juntas, Spanish for "hands together" or "hands joined." He formed the group in 1994 to provide care at medical clinics in Nicaragua and to give scholarships to help Nicaraguan children continue their education. The organization's mission has also grown to providing free care to the poor at a clinic in Oklahoma City, Dr. Shook's hometown, which has seen an influx of Hispanics.

Those efforts have earned Dr. Shook one of this year's ACP-ASIM Oscar E. Edwards Memorial Awards for Volunteerism and Community Service.

Dr. Shook, who manages the ambulatory setting for general internal medicine at the Veterans Affairs hospital in Oklahoma City, first traveled to Nicaragua as a medical volunteer at the request of his daughter, a minister. "And I haven't said 'no' since," he said.

Today, Dr. Shook provides care in Nicaragua during twice-yearly visits.

"There are medical situations there, such as heavy parasite loads, that make episodic care on an annual or semi-annual basis very effective," he explained.

In addition to treating patients who have infectious diseases from parasite infestation, Dr. Shook also sees many patients with urinary tract infections that he said are due to the lack of clean drinking water. "Because they don't have freezing temperatures down there, every day is spring," he said. "There is less of a cyclical nature to the contamination of food and water."

He cannot provide continuing care for management of diabetes, hypertension and heart disease, which are also widespread, so he has developed partnerships with physicians, nurses and community leaders in Nicaragua to provide long-term care. He helps manage those patients long-distance via e-mail and fax.

Dr. Shook said that the availability of medical staff varies depending on the community. One community leader who organizes the details and follow-ups on most of the medical care Dr. Shook is involved with isn't even a physician. "She is an agronomist," said Dr. Shook, "but the need is so great and she does an excellent job."

Medical care, supplies and funding also come from Managua, where a nongovernmental organization coordinates efforts to provide medical care throughout the country.

Back in the United States, Manos Juntas relies on the kindness of local physicians to provide free care, as well as pharmacists who can get the best prices for medications. While Oklahoma City's Hispanic population has grown from 2% to 20% over recent years, the clinic's patient population is now about 70% Vietnamese. To accommodate such a diverse population, there are three sets of signs at the office: one in English, one in Spanish and one in Vietnamese.

Patients at the clinic typically suffer from illnesses such as diabetes and hypertension. When the practice sees problems it can't handle, Dr. Shook often looks to his friends to provide free services. In addition to Dr. Shook, who works part time at the clinic with five other physicians, the clinic also has a staff of nurses, a pharmacist and volunteer medical students.
